Special Instructions
Please make sure to indicate if your dog needs a
Service Animal Certification Application or a
Service Animal Training Permit Application.
If your dog has already been trained to perform as a
Service Animal by you or another trainer, then your dog
only requires a Service Animal Certification Application
to determine if your dog qualifies for ADA Certification.
After you submit your Application Request form, K9 Service Animals will email you the application.
